Outreach Case Manager

Duties and Responsibilities:

  • Responsible for outreach and other engagement strategies to unsheltered and sheltered populations to increase access to, and participation and retention in, harm reduction, case management, treatment, and recovery support services.
  • Specifically responsible to provide outreach and engagement strategies and case

management services to youth and/or their families that have engaged with Grace Harbor.

  • Assists the Recovery Support Specialist to provide outreach and screening to incarcerated individuals who may experience homelessness upon release from a jail and provide a post-release services plan, when needed.
  • Implement referral pathways that engage all demographic groups of the community.
  • Documents services appropriately and timely, using agency approved formats, that clearly describe the need for the service, the intervention provided, the date, actual time and setting of the service, and the individual’s response to the service.
  • Help individuals to create their own recovery plans and develop their own recovery pathways while also helping the person focus on long term recovery, health, and wellness.
  • Engage the patrons with warm handoff to Case Manager and Employment Specialist.
  • Maintain current knowledge of and working relationships with community resources to meet the needs of patrons, such as SEMO-BH’s tobacco cessation program.
  • Submit statistical information and program reports to the Project Director.

Qualifications for Position:

  • Bachelor’s degree in social work, sociology, psychology, counseling, or a related field. Related work experience may be substituted for education.
  • Three or more years of experience in the human services field (case management experience preferred) and one year of experience with the homeless population.

Skills and Knowledge Required:

  • A demonstrated ability to maintain successful working relationships with the individuals served, families, referring agency staff, employers, supervisor, co-workers, and agency staff.
  • Ability to communicate clearly and effectively, maintain confidentiality, and treat everyone with dignity and respect. Able to set and observe appropriate personal limits and boundaries, especially with respect to shelter guests.
  • Knowledge of the characteristics of low-income areas and the community resources in the service area preferred.

Amount of Travel/Other Special conditions:

  • Ability to travel required.
  • Must have a valid driver’s license and daily access to an automobile that is properly licensed/insured.
  • CPR and First Aid certificates preferred but not required.

Supervisory Relationships: This position reports to the Case Manager and has no supervision responsibilities.

Job Type: Full-time

Pay: From $45,000.00 per year
